Payment in full is required at the time of service for all Self-pay patients and for all non-covered services. If Westside Family Medical Center, P.C. does not participate with your health plan, you will also be required to pay in full at each visit.

If we participate with your health plan, you will be required to pay your fixed office co-pay at time of service. If your health plan does not a have a fixed co-pay, we will bill you after insurance has processed the claim for any deductible and/or co-insurance due.

We accept cash, check, money order, MasterCard, Visa, Discover and Debit cards with the Visa or MasterCard logo.

Payment in full is required for the following services regardless of insurance coverage:

• DOT physicals
• School or work physicals
• Sport or camp physicals
• Fees for completion of forms presented by patient
• Medical Supplies provided to the patient for use at home
• Oral medication dispensed in-office

Borgess Hospital owns and operates the lab and x-ray facility in our building. Services provided by this facility will be billed by Borgess Hospital. This will be a separate bill and not included in any fee billed by Westside Family Medical Center, P.C.
